I haven't touched on mean, mode, medium, and range in a little while so I could be wrong, but if (mean) means average, i think average is adding 2 grades together then dividing the number of exams, so the average needs to be 90, so what I did was i found the difference between the original grade on the first exam, so 90-86=4, so i guessed at 94 being the next grade needed. (86+94)/2=90. 86+94=180 then divide 180 by 2 and it equals 90. so yeah your answer is 94.
